File a checklist request
Use this form to collect unpaid balances from fraternity or sorority members. You must enter the student’s first and last name, student ID number, the name of the student’s fraternity or sorority, and the student’s email address to complete the request. Please verify your checklist request meets the Authorized Checklist Requests criteria.
