Release of Conduct Information Waiver (FERPA Waiver)
Student conduct information is considered confidential. There are some circumstances under which a student may wish to share their conduct information with another party, for example: being accompanied by an adviser to a hearing, or permitting a parent to review conduct information. Students who wish to make such a request must fill out the Release of Information Waiver, which can be found here for currently enrolled students and here for formerly enrolled students. This form must be submitted by the student themselves.
Office of Student Conduct Dean’s Form
Beginning July 1, 2021 there will be a $10 charge for each requested Dean's Form. (Ex: If you request to send a dean's form to five institutions, the charge is $50.). If you require more than five Dean’s Forms, you will need to submit a second request. All subsequent requests will follow the same fee schedule. Fee waivers are available if you have a financial need.
Some transfer or graduate school applications and employers require verification of a student’s disciplinary record by the university; this verification is called a Dean’s Form at Indiana University. If you have been asked to provide such information to an external institution or agency and wish for the fee to be billed to your bursar account, please complete this form. This form must be submitted by the student themselves and it requires authentication.
If you are no longer enrolled at IU or prefer to pay via credit card instead of having it billed to your bursar account, submit this form.

Fee Waiver Request

Organizational Amnesty
We believe that student organizations are capable of making good decisions and engaging in a Culture of Care for the health and safety of others. In partnership organization members to promote health & safety, responsibility, and self-governance for a culture of long-term well-being, organizations will be eligible for amnesty from conduct processes in certain situations. Role of Advisor Form
After a student submits the Release of Conduct Information Waiver (FERPA Waiver) to designate an advisor with access to their information, the advisor must then read and sign the Role of Advisor form before engaging in a meeting, hearing, or file review. Complete the form.
